Orthognathic (Jaw) Surgery: Your Questions Answered
Orthognathic surgery, or corrective jaw surgery, realigns the upper and lower jaws to improve bite, facial balance, breathing and speech. Below, orthognathic surgeon Dr Dipesh Rao answers the questions patients in Bengaluru ask most often before jaw surgery.
What is the primary goal of the surgery?
To correct jaw disproportion, improving bite function, facial aesthetics and, in some cases, airway patency. The main aim is function, with a balanced facial appearance as a natural result.
Why do I need orthodontics before surgery?
Braces decompensate the teeth, moving them into the ideal position for where the jaws will be after they are moved. This alignment beforehand lets the surgeon reposition the jaws accurately and gives a stable final bite.
How long is the total treatment journey?
Usually 18 to 36 months, including the pre-operative and post-operative orthodontic phases. The surgery itself is a single stage within this longer, planned journey.
Will my jaws be wired shut?
In modern practice we use internal fixation with small plates and screws, so the jaws are rarely wired shut. Elastic bands are often used to guide the bite during healing instead.
What are the risks of nerve damage?
Lower jaw surgery involves the nerve supplying sensation to the lip and chin. Temporary numbness is common, while permanent numbness is a known but less frequent risk that is discussed fully before surgery.
How long is the hospital stay?
Typically 1 to 2 nights in the ward for monitoring and intravenous fluids, after which most patients recover comfortably at home.
When can I return to exercise?
Light walking is encouraged immediately. Avoid contact sports or heavy lifting for at least 6 to 8 weeks, while the bone consolidates in its new position.
Will the plates and screws stay in forever?
Yes. They are medical-grade titanium and designed to stay permanently unless they cause irritation, in which case they can be removed after healing.
Can this surgery fix sleep apnoea?
Yes. Maxillomandibular advancement (MMA) is a highly effective treatment for improving the airway in obstructive sleep apnoea. Each case needs a bespoke treatment plan.
How do I manage nutrition afterwards?
You will start with a clear fluid diet, then progress to a no-chew soft diet for approximately 6 weeks while the jaws heal, before returning to a normal diet.
Do you use virtual surgical planning?
Yes. We are one of the few centres in the country with in-house virtual surgical planning software with complete planning and design ability, and we perform rotations in cases where there is a need. This gives precise, predictable jaw movements.
